Public bodies may provide their own FOIA forms or an online portal for submitting a request. You can also use this sample form for your request, and deliver it to the public body that has the records you are seeking. However, a public body may not require a request to be submitted on a standard form. You may submit your FOIA request by mail, by hand delivery, or by another method of communication used by the public body.
