Crime Victims Compensation Online Form

If you or a family member has been the victim of a crime, the Crime Victims Compensation Program (Program) may be able to help by reimbursing you for expenses you had because of the crime. You can apply for compensation for: medical, hospital, dental, counseling and transportation expenses, loss of earnings, accessibility, tuition loss, relocation costs, crime scene cleanup, replacement costs, replacement service loss, funeral and burial expenses, loss of support, and dependent replacement service loss.

To receive compensation, you (or your family member) must:

  • Be a victim of a crime that happened in Illinois
  • Report the crime or file an Order of Protection
  • Cooperate with the police, courts and the Crime Victims Compensation Program
  • File the claim within 2 years of the crime
  • Not be involved in illegal activity that caused the crime to happen

There are exceptions to the above requirements (especially if the victim is younger than 18 years old).

This program will not reimburse you for:

  • Pain and suffering
  • Stolen or damaged property

Please note: the overall maximum award shall not exceed $27,000, however monetary limits apply to most benefits.

Your cooperation with the Program and the submission of complete and accurate information will assist us in processing your claim in a timely manner. Making a false statement in this claim form with an intent to mislead the Program is a criminal offense punishable as a misdemeanor under 740 ILCS 45/20 Ch. 70, par.90 (b)
